Terminal-Phase: With blue-green region on the head and on the upper front of its body. Yellowish-pale diagonal stripe behind eye. IP: yellowish with three diffuse grey saddle spots.
Initial-Phase: Frequently pale yellowish and sometimes show three diffuse, grey saddle spots.

Size : 30 cm

Habitat : Inhabits coral-rich lagoons and outreefs, to about 20 m.

Distribution : Indonesia to SW Japan, the Philippines, Micronesia and Samoa.
